Dead Space belongs in both categories.
2 was absolutely incredible and a great direction for the series, very analogous to the Aliens movies.
Similarly to the Aliens movies, the third one was a dumpster fire that tried to be too many things at once.

My main factor in this list is how much the game improved and also how well it built off the foundations of their predecessors.

I can't think of many worst sequels mostly because videogame sequels actually tend to be better or at least on par with the previous game(s). So here's all I got for now.
- Dark Souls II
- Ridge Racer Unbounded (more like a spinoff but I'll count it for now)
- Star Wars The Force Unleashed II

- Hitman Bloodmoney
- God of War II (honestly, I played this for the first time in 2009 and I was blown away)
- Skate II
- Tony Hawk's Underground
- GTA IV
- Uncharted 2
- Uncharted 4
- Spyro 2
- Star Wars Battlefront II (Pandemic developed)
Not only bad sequels, but I passionately h-a-t-e these games
- Hitman Absolution
- SOCOM 4
- Dark Souls II (hate is a strong word, but as time goes on, I teeter towards it)
- Star Wars Battlefront (EA reboot)
- Halo 4
Not bad games, but I expected better (or something more different than the prior entry)
- GTA V
- Black Ops
- Fallout 4
- Halo Reach
I think what makes a bad or disappointing sequel is when the game kinda just misses the point. It gets lost in market trends or has little connecting tissue to it's original identity. And a great sequel normally doubles down on an already solid foundation.
Uncharted 4 is so great because it doubles down on the Uncharted bombast while also taking storytelling and pacing lessons from The Last of Us.
Hitman Absolution is so bad because its only connecting tissue to prior entries is Agent 47 and some parts of some levels have sandbox gameplay. It presents new systems that don't work when mixed with old systems (i.e. disguise system is broken in open world sections).
A disappointing sequel, to me, comes from not really deviating much. Or perhaps, rather than doubling down, they make more of the same (i.e. Skyrim and Oblivion aren't all that different. Even lame combat from Oblivion is mostly unchanged in Skyrim 6 years later).

@scotty Oh yes I forgot to add Hotline Miami 2: Wrong Number to my worst sequels list. A sequel that tried to do a lot more and did much to compromise the original's brilliance.

Just gonna add to this after just finishing it.
Psychonauts 2- Managing to take all the best aspects of the original, making them stronger and tighter (minus a few weird spots of geometry glitching out) adding a strong dash of emotional story telling (that might be me, maybe) and with worlds that have such a massive dose if creativity you can't help but stop for a second and just admire the beauty and insanity of it all.
